APPLICATION CHECKLIST 
God has called you to carry the promise of His grace to a people who haven't experienced the joy of living in Christ. We look forward to working with you as you prepare to GO.

(1)   For Summer Missions: Please submit the online application by the
       December 15, 2006 due date.

(2)   Be sure to print and distribute your four reference forms (found under the
       References tab in the online application) along with confidential
       envelopes that you will provide to the following individuals:
                     Pastor
                     Campus/Church Minister
                     Professor
                     Personal Friend who has known you well
                        within the last two years

      Have references returned to you sealed in confidential envelopes. Submit
      these with your printed application to your BCM Director or Church
     Collegiate Minister prior to your local interview. Consult your local BCM   
     Director or Church Collegiate Minister for local interview dates. Your BCM
     Director or Church Collegiate Minister will forward these to the State
     Office after your local interview. Your appointment will not be confirmed
     until all references are received.

(3)   Submit 3 passport photos to your BCM Director or Church Collegiate
       Minister.

(4)   Complete the following policy agreement, sign, seal and return to your BCM Director or Church Collegiate Minister.
